Forgot User Name – Help

Validate Your Identity

1. Select the applicable Provider Type and click Continue.
2. Enter your Licence Number if you are a Dental Provider, your providerConnect™ Provider Number if you are an Extended Health Services Provider, or if you are a Drug Provider, enter either your CPhA/ ODB / AQPP Number or your providerConnect™ Provider Number.
3. Enter the name of the practitioner exactly as it is shown on your most recent statement (i.e. DR J DOE (no punctuation) or ONTARIO 12345 LTD).
4. If you are a Dental Provider, you will also need to select the Province.
5. Click 'Continue'.
6. You will be required to answer your challenge question.
7. Click 'Submit'.
8. Confirm your Email Address by re-entering it.
9. Your User Name will be forwarded to your Email Address.

Sign In

1. Going forward, you will go to the Sign In area on the Secure Services Welcome Page and enter your User Name and Password to access your account.
